Summary:
The Chaplain is responsible for providing support to hospice Individuals, families, and staff relating to the spiritual aspect of the hospice program.
_____________________________________________________________________________________ Essential Job Functions and Duties:
Provide spiritual support and counseling to Individuals and families.
Arrange for the visits of clergy and other members of religious organizations in the community to Individuals who request such visits.
Advise Individuals, families, and caregivers of this availability.
Develop a comprehensive Spiritual Care Plan based on the interdisciplinary team (IDT) assessment.
Maintain a minimum Productivity Standard according to Autumn Policy.
Productive time consists of face- to-face contact with Individuals and families in the form of direct care activities.
Productive time also includes coordination of care activities with families, physicians, and host nursing facility partners, such as:
TQMs, at-risk meetings, information presentations, or meet and greets.
Assist with bereavement follow-up services with families as appropriate.
Provide spiritual support to Autumn team members, volunteers, and facility staff as needed.
Review and evaluate treatment plans for all Individuals served.
Participate in regularly scheduled IDT meetings to coordinate the care of the Individual and family, exchange information and solve problems, and receive staff support and in-service information.
Communicate appropriate spiritual information to the IDT regarding Individual and family needs.
Maintain all Individual privacy and confidentiality in accordance with Autumn policies and HIPAA requirements.
Complete documentation of all Individual visits on the clinical record per Autumn policy.
Submit all paperwork in a timely manner to update and maintain Individual hospice medical records.
Participate in Autumn quality improvement programs as requested.
Provide community programs concerning hospice service and related issues of illness and loss.
Ensure that all care is provided consistently with respect for Individual rights.
Report any/all allegations of Individual abuse, neglect, and/or misappropriation of property in accordance with Autumn policies.
Follow standard precautions and infection control procedures at all times.
Attend and participate in all in-service trainings and meetings as needed.
Participate in developing and updating Autumn policies and procedures as requested.
Report all accidents or incidents observed.
Identify and respond appropriately to emergency situations.
Maintain required clinical competency/license as appropriate.
Complete other duties as assigned.
Position Qualifications and Entrance Requirements:
Graduate of an accredited School of Theology or equivalent Must have Certificate of Ordination or Endorsement from congregation.
Must have experience working with Individuals and families coping with terminal illness and death.
Must have demonstrable competency with computers (MS Word, MS Excel, etc.
) Experience working independently with minimal supervision is required.
Solid verbal, written, and interpersonal communication skills are required.
Candidates must successfully complete criminal background check and physical (including TB screening) prior to completing Orientation.
Preferred skills and/or
Experience:
Certificate in Pastoral Education with a minimum of three (3) credits preferred Previous hospice experience is preferred Experience in a customer-service oriented environment is preferred Physical Demands/Work Environment:
Must be able to lift up to 50 pounds.
Frequent lifting and/or carrying of objects weighing up to 25 pounds.
Must be able to complete physical activities (walking, standing, talking, hearing, pushing, reaching, seeing, stooping/crouching, smelling, depth perception, identifying colors) as needed.
Must be able to travel to Individual locations on a daily basis Must be able to work with exposure to possibly disagreeable conditions including but not limited to:
odors, infection, illness, and physical contact with disruptive Individuals.
